What should I have ready?
- Your D4 form, with your parts filled in

- A list of your medicines
- Any specialist or hospital letters
Parking and access
Tell us on the free call about parking, stairs or entry codes. We need a quiet room with a chair and a table, and space for an eye chart.

Is the medical the same as at the clinic?
Yes. It is the same DVLA D4 medical, with the same checks and the same eye test. Only the place is different.
